The Mission of Case Management and Outreach at Chase BrextonThe Case Management and Outreach Department, composed of case managers, outreach workers, a social security eligibility specialist and an intensive case management team, is focused upon reducing and eliminating the barriers many patients encounter in attaining and maintaining quality health care. Case Management strives to ensure that all patients - regardless of age, gender, religion, sexual orientation, or socioeconomic status - have access to all the quality services for which they are eligible and entitled. The department is founded in the belief that patient self-determination is the guiding principle when delivering services to patients. We adhere to and base provision of services upon the principles and values established by the National Association of Social Workers Code of Ethics. All new, HIV medical patients should call Case Management at 410-837-2050 ext. 1427 to schedule an initial intake for any center (except Easton). Case Management is available during the same hours that services are offered by the medical staff and also by appointment.
